The secrets to dying well lie in understanding your choices, having your wishes known, tapping into local resources, and making sure those wishes are documented and accessible when it matters most. Join local end-of-life planning consultants for an open, honest conversation about what it means to die well—and live with intention. You will be guided through the tools, decisions, and discussions that help ensure your values are honored, your affairs are in order, and your loved ones are supported. Whether you're planning ahead or supporting someone else, this session offers practical insight and emotional clarity for one of life’s most important transitions.

Presenters: Hilary Walker is a licensed Occupational Therapist with more than 25 years of experience helping people maintain independence and quality of life at every stage. She has worked in home health, acute care, and as the lead of PeaceHealth’s Advance Care Planning program. A passionate patient advocate, Hilary ensures that clients are actively involved in planning for their future. She is now working as a consultant with Known Wishes, and is the co-founder of Stay Home Improvements, a collaborative approach to helping people stay active, engaged and safe in their homes and communities for as long as possible.

Hilary Walker will be joined by Jennifer Carlson, Hospice RN and ACP facilitator, and/or Brian Flowers, funeral director and owner of Wildflower Funeral Concepts.
